The Foucher flap was described by Braun and Foucher in 1979 for reconstruction of thumb soft tissue defects. Modifications including the use of middle finger dorsal skin and sensory transfers using superficial radial nerve branches have been described. We describe a modification using vascularized bone graft from the index finger metacarpal during synchronous index finger ray amputation. The bone was used to perform a secondary reconstruction of a non-union of the thumb proximal phalanx
